update: so here's what we ended up doing. instead of waiting for puppy to wake up and whine to go out and pee I set my alarm to wake me 30 min before he usually gets up. This way, I wake him - I'm not responding to him calling me. Strange, but the first night we did this, it totally worked. I woke him at 3:45, took him out to pee, but him right back in the crate and went back to bed. not a peep. and I'm afraid to jinx it, but its been a week and still working. now I'm going to start moving the time earlier and earlier (slowly!) until he no longer has to go in the middle of the night. we moved to 3:15 yesterday and so far so good.
thanks for all of the support and ideas!

the eating sticks/rocks/leaves is bad again now that the snow melted. I've been bringing some sticks and leaves inside to work on 'leave it' and 'drop it'. he wants those sticks so bad you can practically see him break a sweat trying to ignore them for his treats. outside is a real challenge, but we'll keep working inside too and hopefully he will get it someday.

Dogs do like routine!! I would though not keep putting the time back and back because you could end up that he may need to go again in the night. What time are you hoping to aim for? As for the sticks etc you prehaps should try higher value distractions like chicken or prehaps a favourite toy which unlike a treat is always there
